Family Rapid Rehousing Forms

**Please Note Before filling out the Family Rapid Rehousing fillable forms, download them to your computer first! Then open the form up and fill them out independently. Make sure to always save your work. When finished with a form make sure to hit save. Once completed the below forms and the checklist please email them to us. The forms are hyperlinked below in the checklist.

Dear Applicant,

Thank you for filling out the Family Rapid Rehousing application. You can either fill it out electronically or by hand. If it seems daunting, just take a few breaths and relax. If you have any questions about the application process, please ask Marie! Here are a few things you need to know:

1. The Valley Homeless Connection Assessment and Intake: Begin with Part II. Head of Household (under the second solid black line).

2. The Valley Homeless Connection NO INCOME STATEMENT is to only be filled out if you don’t have any income. If you have income, you will have to provide income statement(s).

3. The HOMELESS CERTIFICATION FORM: Fill out beginning with CHRONIC HOMELESS CERTIFICIATION (under the first solid black line).

4. The Vulnerability Index- Service Prioritization Decision Assistance Tool (VI-SPAT) only needs pages 4-10 filled out.

5. Copy of the rent agreement between landlord and tenant.
